The following forms are available to download as Word documents. If you have any difficulty downloading or using these forms, please contact the ACUC office at 642-8855 and we will assist you.

All protocols must be submitted electronically to the Animal Care and Use Committee either by attachment from the PI's single-user email account to acuc@berkeley.edu or on a CD-ROM (MAC format) to our office. Please note that our computers are unable to read floppy or zip disks.
